Student Covid-19 reporting form

Students should use this form to tell Goldsmiths if they have Covid-19 symptoms, a test result or are self-isolating.

This form will send your details to your academic department, student support and our Estates team. It will also be sent to your accommodation provider if applicable.

We will use this information to make provisions for you and to help protect our community from Covid-19. Either our Accommodation or Wellbeing teams will contact you to provide advice on self-isolation and accessing support.

Collection of information to prevent and manage outbreaks of COVID-19

Students and staff will be asked to provide Goldsmiths with information if they have Covid-19 symptoms, a positive test result or are required to self-isolate. This information will be used in line with Goldsmiths' Covid-19 Outbreak Prevention and Control Plan to prevent and manage outbreaks of Covid-19 and to ensure that staff, students and the community are protected from the impact of Covid-19.

Students and staff will be asked to provide information on:

  • The start and end dates of self-isolation periods
  • Whether they have had a Covid-19 test and if so what the results were
  • Where in the last 48 hours they been on Goldsmiths campus and accommodation

Where necessary Goldsmiths will share information with the NHS Test and Trace and Goldsmiths accommodation providers in order to prevent and manage outbreaks of Covid-19.

The legal basis for processing this data

This information will be processed under the Data Protection Act 2018 and the General Data Protection Regulation. The lawful grounds for processing the personal data are:

  • Processing is necessary for compliance with a legal obligation to which the Goldsmiths is subject
  • Processing is necessary for the performance of a task carried out in the public interest

The processing of special category data is necessary for the purposes of performing or exercising obligations or rights which are imposed or conferred by law in connection with employment, social security or social protection and in line with Goldsmiths' Covid-19 Outbreak Prevention and Control Plan.
