Consent for holding and disclosing your personal data

This form allows you to give your consent to Goldsmiths Disability Service to hold and disclose information regarding the nature and existence of a disability, long-term medical or mental health condition or Specific Learning Difficulty, such as dyslexia on a need-to-know basis.

You can decide the level of detail you wish to be disclosed. For example, you may agree for relevant staff within Goldsmiths to know that you have a disability, but not details regarding the specific nature of your disability or condition. The intention of any disclosure is to ensure relevant Goldsmiths staff understand your disability and how this impacts on your studies. This way, we can support you better.
